Foster Agreement Heath’s Haven determines the criteria for fostering, decides which animals are eligible for foster care, and appoints foster caregivers. Heath’s Haven may refuse any specific request for any reason. We will inform you of any medical treatments to be administered, the anticipated length of the foster period, the objectives of each particular placement (restoring to health, raising to adoptable age, socialization, etc.), and any other restrictions or expectations we may have. You will be expected to keep the animal safe and secure, return it to Heath’s Haven when requested to do so, and not promise the animal to anyone, or imply that you have the authority to approve a potential adoption. Heath’s Haven retains ownership of all animals placed in foster care, and will make all decisions regarding the adoption and placement of the animals fostered. Unless otherwise approved, the foster parent is responsible for providing all food, litter, bedding, and toys for the animal while it is in their care at home. The foster parent is responsible for transporting the animals to and from veterinary appointments, surgeries, evaluations, vaccinations, etc. The foster parent may also be responsible for transporting the animal to and from adoption events. I have read and understand the statement above. I certify that all the information contained in the application is true and correct. I understand that although Heath’s Haven takes reasonable care to screen animals for foster placement, it makes no guarantee relating to the animals’ health, behavior, or actions. I understand that I receive foster care animals at my own risk, and I can decline to accept any animal for which Heath’s Haven has asked me to provide care for. I acknowledge that Heath’s Haven is not responsible for any property damage or personal injury suffered by me, members of my household, including my own animals, or any third parties during foster placement, and I assume liability to provide adequate controls to prevent such damage or injury.The foster parent will not make medical or treatment decisions with out the consent of Heath's Haven Rescue. The foster parent does not have authorization to decline or discontinue medical treatment, physical therapy treatments, or any other long-term treatments for the animal in their care. In case of an emergency, the foster parent will contact Heath's Haven to authorize medical treatment. If Heath's Haven Rescue can not be reached by the foster parent, only emergency medical treatment may be authorized. The foster parent is not able to authorize or consent to euthanasia. The foster parent is authorized to schedule "Meet & Greets" and "Home Visits" for potential adopters. The foster parent is not authorized to make final decisions on the placement of their foster animal. Heath's Haven Rescue will make and attend all final arrangements with potential adopters for the delivery/transportation of the animal. *
